We are investigating a breach of patient data potentially impacting patients served using ConnectOnCall. ConnectOnCall is a telehealth platform and after-hours on-call answering service with automated patient call tracking for healthcare providers. Information potentially included in the breach includes: information shared in communications between patients and their healthcare providers such as names and phone numbers, and may have also included medical record numbers, dates of birth, and information related to health conditions, treatments, or prescriptions. In a small number of cases, Social Security Numbers may have also been impacted.
