Continuum is a provider of health management and patient care coordination services headquartered in Marlton, NJ, near Philadelphia. Consensus “is a leading physician-owned and -governed medical group, comprised of New Jersey-based independent primary care providers and specialty doctors.”
Information included in the breach includes: individuals’ names, addresses, health insurance information, treatment and/or diagnosis code, provider name, and dates of treatment
