The information affected in the breach was from programs Lash Group manages on behalf of Bristol Myers Squibb and/or the Bristol Myers Squibb Patient Assistance Foundation.
Information potentially included in the breach includes: personal information including personal health information was affected, including potentially first name, last name, date of birth, health diagnosis, and/or medications and prescriptions.
