Sound Generations Data Breach Investigation

Barnow and Associates, P.C. is investigating Sound Generations relating to the potential breach of patient, employee, donor, vendor, and volunteer information. The information involved in the incident varies for each impacted group, but may include:

Patients: “name, address, phone number, email, date of birth, and whether or not its clients have health insurance. For those individuals who participated in the EnhanceFitness program, their data may also include their health insurance number. Health history and health condition may also be included for those individuals who provided this information to Sound Generations.”

Employees/employee dependents: “name, phone number, address, date of birth, Social Security number, driver’s license and/or state-issued identification card number, passport number, military identification number, medical information, health insurance information, health insurance policy numbers, digitized or electronic signature, financial aid information, and financial account numbers.”

Donors: “name, address, phone number, email, date of birth, and donation history. Please note that Sound Generations does not collect or store any of our donor’s Social Security Numbers, drivers’ license numbers, financial account information, credit or debit card information.”

Vendors: “name, phone number, address, Social Security number, or Tax Identification number.”

Volunteers: “name, address, phone number, email, date of birth, and drivers’ license number.”
