Information potentially included in the breach includes: names, mailing addresses, dates of birth, and other demographic and contact information, including emergency contact information, Social Security numbers, insurance information, diagnosis and treatment information, and other information from medical or billing records.
Azura is based in Pennsylvania, and operates approximately 70 healthcare locations in more than 20 states and Puerto Rico.
