However, allegations in that case include:
- Claims were often denied without requesting medical documentation or consulting specialists.
- Medical reviewers lacked training in specific medical procedures yet denied claims in as little as four minutes.
- Approximately 25% of denied claims appealed were reversed, showing a pattern of improper denials.
